Appointment Terms & ConditionsAll clients are required to review and adhere to the following Terms & Conditions prior to booking and receiving services with The Aesthetic Vitality.General Services
• A general consultation is required prior to booking any in-person services.
• All services provided are wellness-focused and are not intended to diagnose, treat, cure, or prevent any medical condition.
• Clients are responsible for consulting their licensed healthcare provider regarding medical diagnoses, medications, or urgent medical concerns.
Client Eligibility
• All clients must be 18 years of age or older to receive services.
• A valid photo ID is required at the time of service.
Intake & Documentation
• All required client intake forms and consents must be completed in full prior to appointment confirmation.
• Failure to complete required documentation may result in appointment cancellation or rescheduling.
Appointments, Late & Cancellation Policy
• Appointments are scheduled no more than 14 calendar days in advance.
• A 15-minute grace period is provided. If the practitioner waits beyond this time, a late fee may be incurred or the appointment may be rescheduled.
• Missed, late, or cancelled appointments outside the stated policy window are non-refundable.
• Booked appointments are non-refundable and non-transferable.
In-Home Services
• In-home services include professional travel time and scheduling buffers to ensure service quality and continuity of care.
Phlebotomy Services
• Phlebotomy services involve blood collection only and are not considered an injection therapy.
• Blood samples are collected by the practitioner and sent to independent, CLIA-certified laboratories for processing.
• Laboratory processing times, reference ranges, and results are determined by the laboratory.
• Lab results are reviewed and shared for educational and advisory purposes only.
Supplements & Nutraceuticals
• Supplements and nutraceuticals are purchased separately unless otherwise specified.
• Supplement brands recommended are not manufactured or sold by The Aesthetic Vitality; clients may choose their own brands and follow practitioner-guided dosing.
• Clients are responsible for reviewing product labels and disclosing all medications, supplements, and allergies.
• No guarantees are made regarding supplement outcomes.
Injection Therapy & IV Protocols
• Injection therapies and IV administrations are not performed by The Aesthetic Vitality.
• Clients may be referred to independent medical providers for administration at their discretion.
• Any IV or injection services are governed by the provider’s own policies, procedures, equipment, and medical oversight.
• The Aesthetic Vitality is not responsible for administration, medical oversight, or outcomes related to third-party providers.
Availability & Inventory
• Therapies, products, or services may be subject to availability.
• The Aesthetic Vitality does not guarantee availability or outcomes of any therapy or recommendation.
Telehealth Services
• Clients must provide consent to Telehealth prior to booking applicable appointments.
• Telehealth services are conducted via HIPAA-compliant platforms and may include video, audio, or electronic communication.
• Telehealth has inherent limitations, including the inability to perform physical examinations.
• Consent may be withdrawn at any time by notifying the practice.
Data Use & Privacy
• Client information is handled in accordance with applicable privacy and security standards.
• By signing electronically, clients acknowledge their electronic signature is legally binding.
• This informed consent applies to current and future appointments and is securely stored in the client’s file.
